	Account opening form
	W9 Form
	Board resolution authorizing doing business with ICICI Bank on the business firm’s letterhead.

Documents required for account opening
The documents that need to be submitted will be on the basis of the constitution of your firm.

	Sole proprietorship
	Partnership
	Incorporated companies / LLC
	Government Entities, Affiliates or related non-banking companies

Sole proprietorship

	Either of Certificate of incorporation / Current Tax Registration Certificate / Certificate of good standing
	Sole proprietors’ government-issued photo identification proof – (photocopied front and back to clearly identify all elements of the document ). For list of acceptable ID’s click here

Partnership

	Either of Certificate of incorporation / Current Tax Registration Certificate / Certificate of good standing / Partnership Agreement

	For every authorized Individual operating the account, please provide a government-issued photo identification. For list of acceptable ID’s click here

Incorporated companies / LLC

	Either of Certificate of incorporation / Current Tax Registration Certificate / Certificate of good standing
	Business Resolution or a legal document signifying the share holding pattern of your company on the company letterhead signed off by all directors

	DBA (Doing business as) certificate, if applicable

Government Entities, Affiliates or related non-banking companies
